If you think that you may require any vaccinations relating to foreign travel you need to complete a travel questionnaire in the first instance - ideally at least 8 weeks before you travel.  This can be completed on-line and submitted electronically by clicking on the link below or by collecting a form from the reception desk.

Our travel nurse will then look at your form to determine what vaccinations you require (if any).  You will then be contacted by a receptionist in order to make your appointment.

Generally travel vaccination appointments are on a Thursday afternoon.

Please note that from 1st August 2019, we will no longer be offering private vaccinations for travel purposes.  These vaccinations, if required, can be provided by local pharmacies.

We will continue to offer the vaccines available on the NHS for travel purposes. 

Alternatively you may prefer to make your own arrangements via a private travel clinic, for example the MASTA Clinic in Birmingham (0121 507 8290) or Nottingham (0115 846 8880).  The Unicare Group may also be able to help - tel 01283 260270 or your local pharmacy.
